Our purpose

Making chronic disease care more accessible and effective

My Health Online was created to address a common gap in chronic disease care. Many people only have five allied health sessions each year through a Chronic Disease Management Plan. Without ongoing structure between appointments, care can become fragmented and less effective. We designed our model to ensure those sessions are used in a way that supports meaningful, long-term change.
Our model brings together personalised telehealth dietitian care with a structured Lifestyle Program designed to support change in everyday life. Rather than relying on occasional appointments alone, patients receive ongoing guidance, practical education and follow-up between sessions. This creates coordinated, continuous care that helps people build sustainable habits, not just short-term improvements.

Dietitian-led one-to-one telehealth support

My Health Online’s individual consultations are delivered by accredited practising dietitians via telehealth. Appointments focus on personalised assessment, goal setting and practical support for people living with chronic conditions, and are provided under a Chronic Disease Management referral where eligible.
Care is led by our founding clinician, with additional dietitians joining the team as the service grows.
